Education and Experience:

• Requires Bachelor Degree in Social Work, Counseling, Psychology or other related human services field; must be eligible for a Social Work License.
• Prefers 1-3 year experience with youth/young adults with mental health and substance abuse issues.
• Must have valid driver’s license and proof of automobile insurance as required by law

Job Summary: The Assessment Coordinator is a specialist skilled in gathering significant information through interviews and assessments, facilitating effective responses, preparing and presenting meaningful recommendations in accordance with required timelines. This employee will ensure compliance with safe work practices and maintain safe environments in compliance with established agency standards, licensing regulations and generally accepted practices.

Duties and Responsibilities:

• Respond to all contacts to the Agency regarding its population during work hours and when on-call.
• Determine the nature of all requests for service through telephone and/or face to face interviews.
• Determine through face to face interview and collateral contacts eligibility of any referral for services.
• Facilitate the admission of all crisis support referrals.
• Provide the Agency’s Treatment Management Team with comprehensive report of all contacts, referrals, admissions within 24 hours.
• Respond to community inquiries and requests related to children and family issues and needs.
• Be available when scheduled on-call.
• Direct all potential/real emergencies to network of community providers.
• Be available to screen individual applying/inquiring about service to determine level of need and eligibility.
• Authorize crisis support services when needed.
• Prepare initial screening/assessment for Treatment Management Team.
• Complete comprehensive report detailing client need and strength areas.
• Convene 7-day Treatment Planning meeting.

Required Skills/Abilities:

• Knowledge of local, county, and state resources
• Excellent interviewing crisis counseling, and interviewing skills
• Knowledge of various systems including child welfare, mental health, education, and judicial
• Ability to work flexible hours
• Ability to be on-call evenings/weekends as scheduled
• Experience with Electronic Medical Records Systems
• Ability to work under pressure and multitask
• Proficient and accurate documentation
• Excellent written and oral communication skills
